Information And Referral
A
simple phone call or office visit to the Alzheimer
Resource Center provides a caregiver specific information
regarding the disease and related services in the
community. Referrals made to programs
within our agency or in the community are available
to help the
caregiver better handle their situation.
Caregivers can
obtain information on:
- Adult Day Care Centers
- Assisted
Living Facilities with dementia units
- Community
or in-home respite care
- Home
health care
- Nursing
homes with dementia units
- Other local
community resources
Clients will also receive a follow-up call from one of
our family service professionals to ensure the
referral or information provided was appropriate and
sufficient. |
